For liability purposes, our insurance company requires that we obtain your signed authorization allowing us to have your vehicle in our lot, and to begin the estimating process as soon as practicable. Here's how:

3. Sign and date the form at the bottom

4. Fax it to Hi-Tech Collision CARSTAR at 253.531.2151
